يرجى الاتصال بممثلي QMC (انظر أرقام هواتفهم أعلاه) الذين سيرشدونكم لملء النموذج أدناه. بالنسبة لأهل غزة المقيمين في مصر، نود أن نعلمكم أننا نعطي الأولوية حاليًا لمساعدة الأشخاص ذوي الاحتياجات الطبية العاجلة، والنساء الأرامل والعازبات اللاتي لا عائل لهن، وكبار السن. وبالتالي للمقيمين في مصر والذين لا يخضعون للحالات السابق ذكرها، نعتذر عن عدم استطاعتنا تقديم مساعدات مالية لهم لوجود حالات ذات أولوية أكبر

For Gazans in Egypt, we would like to inform you that we currently prioritize helping people with urgent medical needs, single women with no men supporting them, and elderly people. Therefore, we apoligize for not being able to offer help to people residing in Egypt who are not falling in one of the aforementioned cases.

Notes (Français):
  1. Seulement les adultes musulman.e.s ayant 18 ans et plus peuvent remplir ce formulaire.
  2. Il n'est pas permis de remplir plus d'un formulaire par famille.
  3. La priorité est donnée aux familles avec personnes à charge.
  4. Une fois le formulaire est soumis, vous allez recevoir un courriel de confirmation avec vos informations. Vous DEVEZ retransmettre ce courriel de confirmation à l'adresse courriel: admin@qmc-cmq.com afin d'activer l'inscription.
Notes (English):
  1. Only Muslim adults aged 18 and over can fill this form.
  2. It is not allowed to fill out more than one form per family.
  3. Priority is given to families with dependents.
  4. Once the form is submitted, you will receive a confirmation email with your information. You MUST forward this confirmation email to the email address: admin@qmc-cmq.com in order to activate the registration.

يشترط لملء هذه الاستمارة ان تكون عن المسلمين البالغ أعمارهم 18 عامًا أو أكثر.
لا يسمح بملء أكثر من استمارة للاسرة الواحدة.
الاولوية للأسر التي تعول أفرادا.
